Ceton makes the infiniTV which works with a windows 7 PC. It has 4 tuners and takes a multistream cablecard. This makes for a nice all in one solution. This needs a pci-e 1x slot
On the other hand there is the HDhomerunprime. Which accepts a cablecard and pumps the signal over ethernet to a wwindows media center PC (use win7, win8 is still in beta).
I use the Ceton, it's fantastic, it was a nightmare to set up with Cablevision and every tech they send out has no idea how to use a cable card. You don't have to pay monthly except to rent the cablecard, which for me is $2.
Be sure you give the cable cards enough horsepower (fast processor) because it's a very demanding process on the processor (at least watching over your LAN to media extenders (xbox 360s).
So I pay for one cable card, and have 360s wired throughout the house. I love the set up and works fantastically, as long as you're ready for a long set up process because no cable worker knows how to set up a cablecard.

Or pony up for a TiVo, but that's basically just getting the box from the cable company and paying elseware and still not getting on demand stuff.
