usually happens when it lags in posting a comment and post comment is hit a second time, or poster hits button more than once accidentally (double click). most double posting can be explained as mouse actuator error

unfortunately... I need a way to provoke this bug in order to fix this. Seeing the symptoms won't help a bit :(
