Photo of myself and a lot of time with the clone-tool in photoshop.
First I cloned the whites of my eyes to create the triangle shape. Then, I clone the original iris/white transition onto the edges of the triangle. After that, some shading was needed on the whites to get the correct shape, but not much. All the highlights and reflections were from the original photo.
In hindsight, I would have taken some pictures with my eyes looking to the far right/left to give more unobstructed white to clone from.
